This questionnaire will be used to help build meaningful and caring relationships between athletes, parents/caregivers, and coaches. This questionnaire is by no means intended to label, limit, or otherwise inhibit any athlete interested in the sports of gymnastics, dance, and/or karate. The information contained within the questionnaire will not be shared with any outside source, any other athletes/families, nor any non-essential coach or employee within MHG. As you complete the questionnaire, please know that there are exceedingly few situations that could preclude an athlete from flourishing as an MHG athlete.
