This application is to be completed by all applicants who desire to serve in any position (volunteer and/or compensated) involving any access to children (minors under 18 years of age) or developmentally disabled persons in LIFECHURCH7 sponsored events/activities and ministries. It is being used to help LIFECHURCH7 provide a safe and secure environment for those children, youth, and developmentally disabled persons who participate in our sponsored events and programs and use our facilities. This application will need to be renewed at least every five (5) years. Upon completion you will be emailed a background check. Our background checks are completed by a third party and will check for criminal history.

I understand that this background report is for the purpose of investigating any incidents of workplace misconduct or criminal activity.
