Donation Request Form

Beyond Juicery + Eatery is committed to the communities we serve and are pleased to contribute to worthy causes and fundraising events in those communities.

In order to be eligible for a charitable donation, your organization must meet the following requirements:

1. Submit your request at least four weeks prior to the event.

2. Not be affiliated with any controversial cause or group. All groups will be reviewed by and at the discretion of The Company.

3. Provide a Federal ID that indicates a charitable organization.

4. Provide a physical mailing address for vouchers to be mailed if approved.

We receive a tremendous number of requests and will process your application as soon as we can prior to the event date given the above requirements are met.
