Note Taker Request Form

Access to a Volunteer Peer Notetaker: Assistance is determined on a class-by-class basis and is dependent upon the nature of each course and the method of instruction. This accommodation is not considered a substitute for a student’s full participation in class. Should a Volunteer Peer Note taker be put in place they will be asked to provide their own notes to be shared when the course slides and/or lecture notes might not be sufficient. Students should read and review the course syllabus and attend the first three class meetings before submitting a Note Taker Request Form. OAS may need to consult with faculty regarding the logistics of their course.
